Patient medical documentation – What data do we need?
To properly assess your symptoms, we primarily need the results of tests already performed, including laboratory tests (e.g., blood counts, ESR, CRP, urinalysis, immunological tests, etc.), microbiological tests (including cultures), histopathological tests – especially biopsy results and descriptions (e.g., skin samples, polyps or intestinal lesions, lung biopsies, etc.), imaging tests (X-ray, ultrasound, CT, MRI, echocardiography, scintigraphy), additional tests (e.g., ENG, EMG, EEG, ECG [including Holter results], spirometry, and others), genetic testing (e.g., WES), and screening tests for rare diseases (e.g., dry blood spot test – DBS). We also need descriptions of diagnostic procedures such as gastroscopy, colonoscopy, or bronchoscopy (as well as others).
Also crucial are your previous consultation results and hospital discharge summaries.
Various types of photos are another important part of the documentation. These may help us assess visible changes (e.g., skin lesions), and when sent over time, they allow us to observe their development or transformation.

Data security at Saventic Care
All data shared with Saventic Care is securely protected. It is transferred via a specially designed Saventic Care application and secured with passwords. Only authorized physicians, bound by medical confidentiality just like in a standard doctor’s office, are allowed to access the data. If you decide not to continue with Saventic Care’s assistance, you can also withdraw your application and request deletion of your data from our system. The process is very simple – just send us an email request.
